The Three-Agent Cleaning System USP 797 Requires — Cleaner, Disinfectant, and Sporicidal

One of the most consistently cited deficiencies during state board of pharmacy inspections is inadequate cleaning and disinfection practices in compounding environments. And one of the most common root causes of those deficiencies is a fundamental misunderstanding of what USP 797 actually requires — specifically, that effective surface decontamination in a compounding pharmacy is not a single-step process. It requires three distinct types of agents, each serving a different purpose, applied in a defined order and at defined frequencies.
USP 797 Table 9 defines these three agent types — cleaner, disinfectant, and sporicidal disinfectant — and Table 10 establishes the minimum frequencies at which each must be applied to each surface in your classified areas. Understanding the distinction between these agents and the specific requirements that govern their use is essential for every compounding pharmacy director and quality manager.
The Three Agents — What Each One Does
USP 797 Table 9 defines the purpose of each agent type clearly. They are not interchangeable — each addresses a different category of surface contamination:
Agent 1 — Cleaning Agent
A cleaning agent is typically a surfactant-based agent used for the physical removal of substances from surfaces — including dirt, debris, microbes, and residual drugs or chemicals. The purpose of cleaning is removal, not killing. A cleaning agent loosens and removes contaminants so that subsequent disinfection can be effective against the surface itself rather than against a layer of organic matter that would otherwise shield microorganisms from the disinfectant.
Agent 2 — Disinfectant
A disinfectant is a chemical or physical agent used on inanimate surfaces and objects to destroy fungi, viruses, and bacteria. Disinfectants work by killing or inactivating microorganisms on a cleaned surface. USP 797 requires the use of an EPA-registered disinfectant — or equivalent for entities outside the United States. Importantly, some EPA-registered one-step disinfectant cleaners are formulated to accomplish both cleaning and disinfection in a single application — but this does not eliminate the requirement to also apply a sporicidal disinfectant separately.
Agent 3 — Sporicidal Disinfectant
A sporicidal disinfectant is a chemical or physical agent that destroys bacterial and fungal spores when used at a sufficient concentration for a specified contact time. It is expected to kill all vegetative microorganisms. Spores are the dormant, highly resistant forms of certain bacteria and fungi — including organisms like Clostridium and Bacillus species — that can survive standard disinfection processes that effectively kill vegetative cells. A sporicidal agent is the only way to address this category of contamination risk.
🧪 Why all three are required: A disinfectant that kills bacteria and fungi in their active vegetative state will not necessarily kill their spores. A cleaning agent removes contamination but does not kill microorganisms. Only by using all three agents — in the correct order — can a facility address the full spectrum of contamination risk on cleanroom surfaces.
The Required Sequence — Clean Before You Disinfect
USP 797 is explicit about the required sequence: surfaces must be cleaned prior to being disinfected. This sequence is not arbitrary — it reflects a fundamental principle of surface decontamination. Organic matter on surfaces — residual drug, debris, skin cells — can shield microorganisms from disinfectants, significantly reducing their effectiveness. Cleaning first removes that organic burden and allows the disinfectant to contact the surface directly.
Key sequencing requirements from USP 797 include:
- Cleaning must be performed in the direction of clean to dirty areas — moving contamination away from the most critical zones
- The same floor mop may be used in both the buffer room and ante-room, but only in that order — buffer room first, then ante-room
- Mops used in areas where hazardous drugs are compounded must be dedicated for use only in those areas
- When sterile 70% IPA is used as a disinfectant, it must be allowed to dry completely before proceeding
Minimum Frequencies — USP 797 Table 10
USP 797 Table 10 establishes the minimum frequencies at which cleaning, disinfecting, and sporicidal disinfectant application must occur for each surface in your classified areas. These are minimum requirements — your SOPs may specify more frequent cleaning based on your facility’s risk assessment and quality program.

*Ceilings of the SCA are required to be cleaned, disinfected, and applied with sporicidal disinfectant only when visibly soiled or when surface contamination is known or suspected.
⚠️ The sporicidal requirement is not optional: A sporicidal disinfectant must be applied — at minimum monthly for Category 1 and 2 facilities, and weekly for Category 3 facilities — to all required surfaces. A facility that uses only a standard disinfectant without a dedicated sporicidal application does not meet USP 797 Table 10 requirements, regardless of how frequently it cleans and disinfects.
SOP Documentation Requirements
USP 797 requires that all cleaning and disinfection activities be governed by written SOPs. These SOPs must specify:
- The specific cleaning, disinfecting, and sporicidal agents approved for use at your facility
- The frequency of application for each agent at each surface location — consistent with Table 10 minimums
- The method of application for each agent
- The manufacturer’s required contact time for each disinfectant and sporicidal agent used
- The direction of cleaning (clean to dirty)
- Garbing requirements for personnel performing cleaning activities
Personnel must be trained on any changes to cleaning and disinfecting procedures. All cleaning, disinfecting, and sporicidal applications must be documented according to the facility’s SOPs — meaning a cleaning log that records what was cleaned, when, with what agent, by whom, and that the required contact time was observed.
🚨 Common inspection finding: Facilities that have cleaning SOPs but no documentation logs — or logs that are incomplete, inconsistent, or clearly filled in retrospectively — face serious inspection consequences. The documentation of cleaning is as important as the cleaning itself. An undocumented cleaning event is, from a regulatory standpoint, an event that cannot be verified as having occurred.
Contact Time — A Requirement That Is Frequently Missed
One of the most frequently overlooked aspects of USP 797 cleaning requirements is the mandate to follow the manufacturer’s required contact time for each cleaning, disinfecting, and sporicidal agent used. Contact time — the period during which the agent must remain wet on the surface to achieve its rated efficacy — varies significantly between products.
A disinfectant wiped on and immediately wiped off has not been applied correctly — it has not had sufficient contact time to achieve its rated kill. This is particularly important for sporicidal agents, which typically require longer contact times than standard disinfectants to achieve effective spore kill.
Your cleaning SOPs must specify the required contact time for each agent used, and your cleaning logs should document that contact time requirements were observed.
The Connection to Your Viable Air Sampling Program
There is a direct connection between the rigor of your cleaning and disinfection program and the results of your viable air sampling program. Facilities that consistently apply the correct agents in the correct sequence at the required frequencies — and document that they have done so — are the facilities that tend to maintain clean viable air sampling results between certification visits.
When viable air sampling results begin trending upward or exceed action level thresholds, cleaning and disinfection practices are always among the first areas to investigate. Deficiencies in sporicidal application frequency, agent contact time compliance, or cleaning sequence are common root causes of microbiological contamination trends in compounding environments.
